Imaging endpoints in neuro-oncology trials

Standardized RANO 2.0 Assessment for Glioma and GBM Trials

In glioblastoma and glioma trials, accurate image interpretation is critical to the assessment of treatment response. On conventional MRI, treatment-related changes can overlap with tumor progression, creating challenges for the consistent assessment of disease status across timepoints and readers.

RANO 2.0 provides an updated framework for response assessment, but its consistent application requires expertise in neuro-oncology imaging, standardized acquisition and experienced subspecialist readers.

IAG supports neuro-oncology studies from imaging strategy and protocol development through site qualification and centralized image review. Led by specialist neuroradiology expertise, we deliver standardized, auditable assessments through DYNAMIKA™ across the full imaging workflow.

What our neuro-oncology imaging team does

Neuro-Oncology Imaging Capabilities

Imaging charter and sequence harmonization, site qualification, subspecialist reader qualification and adjudication — with advanced MRI and retrospective RANO 2.0 re-evaluation where your dataset supports it.

Advanced imaging for treatment-related changes
DWI, perfusion and DCE-MRI, with molecular imaging where appropriate, can support the assessment of tumor progression and treatment-related changes, including pseudoprogression.
RANO 2.0 retrospective analysis
Re-evaluation of existing imaging datasets using updated response assessment criteria, where image acquisition and available data support retrospective application.

Neuro-Oncology Indications We Support

Primary brain tumors, brain metastases, CNS lymphoma and pediatric brain tumors, with read methodology matched to enhancing and non-enhancing disease assessment.

Glioblastoma
In GBM, post-treatment enhancement can mimic tumour growth and cost a trial its endpoint. We apply RANO and RANO 2.0 through subspecialist neuroradiology review, standardize MRI sequence selection across sites, and add diffusion and perfusion measures to support assessment of pseudoprogression.
Diffuse Gliomas
Lower-grade gliomas change slowly, so sensitivity to small structural change matters more than speed. We standardize longitudinal MRI acquisition and support volumetric assessment of non-enhancing disease under RANO 2.0.
Brain Metastases
CNS response often needs to be reported separately from systemic disease. We define lesion selection and slice thickness requirements for small metastases and manage paired CNS and systemic reads within one blinded workflow.
Primary CNS Lymphoma
PCNSL responds rapidly and is steroid-sensitive, making acquisition timing critical. We fix imaging windows and contrast protocols in the charter and use neuroradiologists experienced in CNS lymphoma assessment.
Meningioma
Slow-growing extra-axial tumours require reproducible measurement over long follow-up. We standardize MRI acquisition and support volumetric assessment to detect change reliably across years of imaging.
Paediatric Brain Tumours
Paediatric CNS trials need age-appropriate protocols and criteria built for children. We apply RAPNO, design acquisition that manages motion and sedation requirements, and use paediatric neuroradiology readers.
Spinal Cord Tumors
Cord lesions are small and prone to motion and susceptibility artefact. We specify dedicated spinal sequences and positioning in the charter and centralize review to keep measurement consistent across timepoints.
